Notification of Data Security Incident
April 28, 2022 – On February 27, 2022, our practice experienced a network intrusion that affected
some of our systems. Upon discovery, we immediately suspended our network and engaged a
third-party forensic firm to investigate the incident. After a thorough investigation, we confirmed
that a limited amount of information may have been accessed in connection with this incident.
There is no indication that any information has been fraudulently misused at this time or will be in
the future.
Following a thorough analysis, the investigation determined that information contained in the
affected files and data may have included a limited number of patient’s protected health
information. The type of information contained within the affected data primarily includes patient
names and one: DOB, address, dental chart, and bank account number.
At this time, we are not aware of any evidence to suggest that any information has been misused.
However, we have been unable to rule out the possibility that the information could have been
accessed. Therefore, in an abundance of caution, we are notifying potentially impacted individuals
of this incident.
In response to this incident, we have partnered with the forensic specialists to evaluate and
reinforce existing security measures within our network and facility to ensure optimal data
security. Although we have no evidence of actual or attempted fraudulent misuse of information
as a result of this incident, individuals are nonetheless encouraged to monitor their account
statements and explanation of benefits forms for suspicious activity and to detect errors.
We have established a hotline to answer questions about the incident and to address related

through Friday, 9:00 a.m. to 5:00 p.m., CST.
